How we work inside facility settings

Bluebird partners with assisted living communities, memory care units, skilled nursing facilities, and inpatient rehab settings across Oakland County. We bring the Medicare hospice benefit to your loved one inside the place they already call home.

Will hospice conflict with the care my loved one already receives?

This is the question every facility-based family asks. The answer is no. Hospice is layered onto the care your loved one already has — it doesn't replace the facility staff, and it doesn't disrupt the routines or relationships that are already in place. Facility nurses and aides keep doing what they do. Bluebird adds the clinical and emotional support that hospice covers: pain and symptom management, equipment, medications related to the diagnosis, a chaplain, a social worker, and a 24/7 nurse line.

How Care Works in a Facility

How Bluebird and your facility coordinate

The first 24 hours

A Bluebird nurse meets with your loved one, the family, and the facility's nursing leadership. We review the care plan together. Nothing gets changed without the family and the facility on the same page.

Defined visit schedule

Bluebird's RN, CNA, social worker, and chaplain visit on a regular schedule. Facility staff know when we're coming. The family knows when we're coming. The schedule flexes as needs change.

Equipment and medications

Hospice equipment beyond what the facility provides — specialty mattresses, oxygen, comfort kits — is delivered directly. Medications related to the diagnosis are coordinated and delivered.

Pain and symptom management

Symptom changes are managed by the Bluebird team in coordination with the facility nursing staff. Breakthrough symptoms after hours go through our 24/7 nurse line.

Communication back to the family

Every visit generates an update. Families who can't be present every day get a clear, calm picture of how their loved one is doing.
